Project Description

Summary:  View help for Summary The American Institutes for Research (AIR) will develop a research-based OUD assessment and referral toolkit to support primary care clinicians, rheumatologists, physiatrists, pain specialists, and rehabilitation providers treating people with disabilities and chronic pain related to musculoskeletal conditions. The project also will identify barriers and facilitators to OUD treatment for people with disabilities and will identify approaches that providers diagnosing OUD can use to help people with disabilities transition into an office-based setting for medication-assisted treatment (MAT) or an intensive outpatient or residential treatment program that incorporates pharmacotherapies for treatment of OUD.

The study objectives are to:
  • Identify barriers and facilitators to assessment and risk stratification for OUD in people with disabilities from chronic musculoskeletal pain and factors that inhibit their entry into OUD treatment services
  • Identify evidence-informed practices, instruments, and resources for assessing OUD in people with disabilities from chronic musculoskeletal pain, risk stratification for OUD, and facilitating entry into OUD treatment services
  • Develop or adapt and test a screening instrument for OUD in people with disabilities experiencing chronic musculoskeletal pain
  • Develop a research-based OUD assessment and referral toolkit to guide providers treating people with disabilities related to musculoskeletal conditions and to inform people with disabilities about OUD, OUD risk when taking opioids, and the OUD screening and treatment process
  • Disseminate the issue briefs and OUD assessment and referral toolkit through Webpages hosted by the AIR Center for Multi-System Solutions to the Opioid Epidemic, along with partners that influence the target audiences (people with disabilities, providers, and policymakers).

Universe:  View help for Universe Adults with arthrititis and chronic pain, who are physically disabled due to arthritis for 12 or more months, and are taking opioids to manage arthritis pain or have history of opioid use disorder treatment.

Collection Notes:  View help for Collection Notes We conducted semi-structured interviews with 24 specialists and primary care providers, 24 people with disabilties (PWD), and 8 peer recovery coaches from February-June 2019. The team also tested the Current Opioid Misuse Measure (COMM), an instrument designed for monitoring people with chronic pain on long-term opioids, with 318 people with disability and arthritis who are taking long term opioids from February-June 2020.
